HOW TO SCHEDULE ONLINE

1. Click button: VIEW CALENDAR & SCHEDULE ONLINE

Acusimple Online Scheduling & Patient Portal page will open in new tab.  View calendar & appointments by selecting the Schedule Tab. 

2. Create New Account

Create Patient Portal account with your email address & choice of password.  Required to confirm appointment online.  Use Portal tab.

3. Login to Patient Portal

From here you can:  Schedule appointments, fill out forms, edit contact info., manage upcoming appointments, review documents, exchange secure messages, or email practitioner.

4. Click on Schedule Appointment

Select the type of appointment you would like.  You can view available days & times without creating an account.  

5. Select Day

Available dates are highlighted on calendar.

6. Select Appointment Time

Not finding the time you need?  Secure message via patient portal, email, call, or text.  Appointments are available outside of regular business hours by request.

7. Confirm Appointment

If you have not created a patient portal account, you will be prompted to now.  You cannot schedule your appointment online without one.

8. Appointment Confirmed

A confirmation email will be sent to you.  You can book additional appointments from here as well as the portal tab.

Check the email account you provided to set up your patient portal for the confirmation email.  Or check in your Patient Portal under the Appointments tab or Messages tab.

You will also receive an email and/or text reminder 24 hours before your appointment.  You can edit your preferred contact method in your Settings. 

Need to Cancel/ Reschedule?

Login to your Patient Portal.  If already logged in, click the Portal tab followed by the Appointments tab.  Select Reschedule or Cancel.

Securely Message Or Email Practitioner

Click Secure Message or Email Your Practitioner highlighted in blue & located on the right side of your portal page.  To check sent & received messages, select Messages tab.  

Online Forms

1. Patient Portal

Once logged in, make sure you have selected the Portal tab.  Below is another tab menu, select Forms.

You only need to complete forms pertaining to the service you are scheduling.  Click the button below to go to the list.

3. Edit Forms

To Complete/ Edit/ Update the form click the corresponding button.  Save Draft to continue form at another time & Submit Form when completed.

4. Forms Required

Each form has an indicator before the file name.  All Required means the form is required for any service.  Acu Required forms are required for Acupuncture services.  Massage Required are for massage services  Additional are for services requiring a separate waiver for either Massage or Acupuncture.
